Many intriguing new concepts have emerged regarding the contributory role of bile salts in health and disease. This review is concerned with the physicochemical properties of bile salts and the regulatory effects of bile salts on their own hepatic synthesis and on the hepatic and intestinal synthesis of cholesterol. The quantitative aspects of bile salt secretion and recycling in the enterohepatic circulation are discussed with particular reference to the genesis of cholesterol gallstone disease. Although the terms bile salt and bile acid are used interchangeably by some authors, all physiological functions are performed in the salt (amino acid conjugate) form. Thus when not specifically referring to the chemical pathways of their synthesis, the term bile salt is used in this review in discussing synthesis and secretion of cholesterol and pathogenetic aspects of gallstone formation.
